This is where you will briefly introduce the problem you are working on. One to two short paragraphs should work. Also, before you begin, review the Rubric “Critical Elements” and understand the difference between “Exemplary”, “Proficient”, “Needs Improvement” and “Not Evident”. I used the Critical Elements to grade the papers. This is why each of the Templates are set up with headings (below) that correspond with the Critical Elements for that specific assignment. Please leave the Bold Headings (below) in the paper. You’ll appreciate why in Week 8.
Problem: Provide Contextual Basis
Comprehensively provides a contextual basis for the organizational problem that was chosen, including how the problem fails to meet quality or regulatory requirements. So for example you will provide a brief summary about why the problem you selected is a problem.
Problem: Articulate Organizational Challenges
Clearly articulates organizational challenges posed by the problem. In this section you will discuss the challenges that are present because of the problem you selected.
Support: Provide Data that Supports Existence of the Problem
Provides data that supports the existence of the problem. This is where you might need to do some research. You MUST provide DATA that supports the existence of the problem. You will either need to provide data from your organization or you can retrieve data from the Internet.
Support: How Problem Has Been Addressed In The Past
Thoroughly describes how this problem has been addressed in the past, including the information management systems or patient care technologies utilized, and supports answer with peer-reviewed literature. This section is about what has been done in the past to address the problem. Were there new policies established or new processes implemented?
Support: Relevant Accreditation, Safety, Compliance Standards
Clearly discusses relevant accreditation, safety, and compliance standards, as well as quality initiatives, including how these standards promote a culture of safety within the department, and cites appropriate standards. This section is where you would discuss any accreditation, compliance, safety and quality initiative. For example, you might be able to discuss The Joint Commission Standards, CMS regulations or State law that might regulate your organization. If there is no accrediting or surveying body or State law, then you would need to state that but still discuss quality initiatives and how these promote a culture of safety
SPECIAL NOTE: Critical Element #6 (Articulation of Response) is (Proficient) is a “submission has no major errors related to citations, grammar, spelling, or syntax (the grammatical arrangement of words in sentences)” That typically means 3-4 minor errors. (Exemplary) is a “submission is free of errors related to citations, grammar, spelling and syntax and is presented in a professional and easy-to-read format.” This means no errors in citations, typos, grammar, punctuation, etc. THIS SECTION AND THE SECTIONS ABOVE DESCRIBING THE CONENT SHOULD BE DELETED PRIOR TO SUBMISSION.
